At first, when it was released, I'm pretty sure mostly adults played it, because Penumbra wasn't really that popular, and most players who liked Penumbra were mature. But after it got popular on youtube, a wave of millions of 12 year old pipsqueaks got into it too.
So yeah, most of the players are obviously teens.
I never got that concept about forcing itself to be immersed during playing a game. Basically I don't know how one is supposed to do that due I have enough strong sense of "it's just a game".
However Amnesia game is more about the tension than actual scariness.
I just googled all 4 torture methods on A:TDD (Judas Cradle, The Breaking Wheel, Iron Maiden, and Bronze bull.) on Google. Surprised with what i found, is that the Breaking Wheel is still widely used in a number of countries.
A different version of iron maiden is being used aswell.. It's without spikes, just an iron coffin with very very little space. Here's an article in which the victim describes how it was like
