Delete the settings file shown in this post http://www.frictionalgames.com/forum/thread-17369-post-157685.html#pid157685 and see if that helps.

Can you run the game as adminstrator and see if that fixes it. The only thing I can think of is that when it tries to create a new cache file for the first level it fails as it does not have the correct permissions to do so and that is why it crashes.
