This has been bugging me for a long time

What is that thing on the Desktop icon of Amnesia!

Actually this is not a boxart, this is an artwork from the dates long before this project wasn't even named. It was known as Unknown; and this artwork was there.
If I'm to guess, I'd say a grunt Tongue

Its on a Box Art site, so that's why I said that Tongue
