there doesn't seem to be any use for the padlock anywhere in the incinerator level..at least none that I discovered.

If there was a Good+ ending that we the players hadn't figured out, I sure as heck wouldn't have been the smartie pants to get it when no one else did. I didn't even find 2 of the secret items in Black Plague.

I do enjoy the idea that the item does somehow lead to a better hidden ending in some way the world has not figured out.

I only found 4/10 special items in Black Plague. :S I have only played it once though...

That's an interesting idea to think about... The 'Good+' ending. What if Frictional decided to do something quite original, and put something in the game that is so small, or so well hidden, and then after the game's hype finally levels out a bit, they start hinting at it and bring it up to get everyone to start playing it through again with newfound enthusiasm? Or they do it and don't say anything about it for a long while just to see if anyone out there can actually find it on their own?

That would be quite interesting....
