Here's 1 idea, David was still troubled by the death of his parents, and knowing Grace commited a double murder and having to send her to the mental hospital made things worse. That doesn't however explain what went on on that crazy asylum and the game ending wont match with what i said.
Unfortunatly for me, it seems im getting even more away from the real story and making up my own.

Well, you could get very close but you have quite a wrong base. I didn't borrow anything from Shutter Island and that thought misled you. DamnNoHtml made the same mistake too. It might be due to missing some flashbacks. Nevertheless, although Russ Money found all the notes and flashbacks, he couldn't reveal everything either.

Tanshaydar, tell me if I'm at least close on this one: David and Grace had some sort of relationship (married presumably), but Grace has a split personality. Grace commits a double murder, and is put into a mental institution for rehabilitation/imprisonment. David stops visiting, possibly from him beginning to see Sofia due to his lack of presence around Grace anymore (I think this because Dr. Patrick said that David being around Grace suppressed Sofia for quite some time, so I would assume her being isolated in a mental hospital where she can only see David through glass would have an adverse effect, and increase Sofia's control.), and this allows Sofia (Grace's split personality) to emerge. Dr. Patrick and Dr. S try to suppress Sofia once more, seeing the danger in a psychotic genius (Sofia was obviously extremely intelligent.), but Sofia gains control of the hospital and kills the doctors. David comes to visit, after a long period of not visiting (I'd like to say specifically after a long period of time, because he remembers going there for the first times, but does not remember when Dr. Patrick and Dr. S are not there. According to Ribot's Law, he'd remember the memory farther away than the one that happened sooner, the one "sooner" event being the one that led to his initial capture.), which makes Sofia/Grace capture him, possibly of desperation. Sofia keeps him alive because Grace still wants to be with David, and Grace/Sophia use the same treatment that the doctors did to Grace to get rid of Sofia to get rid of the memory of Grace committing a double murder. Oh, and those water monsters? The fire (monster) is the traumatic experience that may have began to cause this amnesia, because David's family died in a fire while he was young and he was the only survivor.
(I'm guessing more or less here, but I think the place that the fire had taken place in had a lot of chains and metal platforms, since they're pretty common.) Also, I'm pretty sure that ghost that you usually see around is of Dr. Patrick. Maybe he's trying to lead David to the clues that allow him to recollect his memories in the first place? The "second reality" he sees is probably him either asleep or in some coma-like state (Hypnosis, maybe? It is a practice used in these types of matters.), which allows him to have these unrealistic landscapes and creatures, such as the eyeball walls or the blood red ground/sky. I'd also like to add that I think the ending is a memory, and David has been locked in there for quite some time and that is why he is remembering now, or he may be trapped there forever and that is why he is remembering. That's all I can theorycraft for now, I've never really gone into such a deep theory about a story before ever. Good job Tanshaydar. ^_^

You are quite close, but I see people having the misunderstanding that there is apparently "two" reality.
No, there is not just two reality.
Also, in the letter Dr. Patrick wrote to Dr. Selwyn it is stated by Dr. Patrick that Grace and Sofia are two different individuals.

Although Ribot's Law has a very big importance in the story, the actual base is that Dr. Patrick leads a study, which is 'basically' memory replacement, where he mentions the root of the idea in his note titled Regression.

I'm glad you enjoyed and I'm glad you have come with your interpretation, which is very close to what I wrote. If you liked to have deep theories in gaming, I'd suggest you playing Silent Hill series, especially 2. I had lots of references to Silent Hill 2.
