DONT FLY OFF AT ME! : Remove the head of every suit of armor in the game. (This seems to be a common thing among the custom stories, and I see people take the heads off all the time )

That was close - Succesfully hide from a grunt or brute.

These are all those lame type of baby achievements that shouldn't even exist. Basically they give you achievements for playing the game, which is the stupidest type of achievement ever.

I'm impressed with some of the ingenuity of these Achievements.

Came up with some more...

You're grounded! - Hide in a dark corner to avoid detection from an enemy.
Too afraid to look? - Look away as an enemy passes you by without detecting you.
Paralyzed by fear - Do not make a single movement for 5 minutes.
Final lap - In the sewer part where you have to lure the grunt from a critical path, make him do an entire lap of the area.
I just want to be your friend! - Bump into the grunt by accident.
Not another puzzle... - Spend more than 30 minutes trying to solve a puzzle.
I think I lost him - Look back while being chased, only to find out the monster is right behind you.
No, you don't! - Slam a door in the grunt's face.
BOOM! You're dead. - Get killed by the explosion you create in the storage area.

Some might have already been mentioned. Most of them are not good, I think. But I tried.

I think this was a legit achievement on a xbox game, its pretty sad what people will call a 'achievement'
