Gameplay > realism. Some of the light sources you could in theory pick up would last too long to make the element of losing sanity in the dark critical. You would probably finish the game by just lighting one torch in the beginning and moving it wherever necessary. It could also make the game's design too complex to handle if they had to make static light sources wear out in time also, meaning you'd constantly have to find more sources even after you've lit up a room because it would go dark with you gone (and then you'd also ask them to add an element where you can put out lights you don't need right now as to not waste them), and then they'd have to balance the available objects and tinderboxes and every single light source from the very beginning right to the end even more. The right hand is just an icon indicating interaction. Just imagine he's using both. Yes, even if the lamp is still in view (though I suppose it wouldn't be too hard to make it go out of view when carying stuff, but then you'd be in the dark every time you interacted with an object in that way, even if in real life you could probably hold it so the lamp hangs from a finger or two regardless, but that sort of thing is too complex to animate with the free movement of the objects)... The crates can be full and still be light. Other heavier objects you can't throw, just push and pull. Also, it's a video game and it's from a small developer so they focus on what matters most instead of waste resources.
