I don't think that this may fix everything, but you have two "Note_letterthree_Text". (Change it to "Note_letterfour_Text".)

Edit:
Oh, I see now:
"<Entry Name="Note_letterfour_Name"note_letterfour</Entry>"
should be
"<Entry Name="Note_letterfour_Name">note_letterfour</Entry>"
and you forgot that tag for letter three as well.
