EDIT:
This is swisscheese's brother. I was able to get the game running. I re-downloaded the game, deleted all Penumbra Black Plague files in My Docs, reinstalled nvidia drivers, and left the video settings at the default settings. The game loads fine now.

EDIT 2:
The game works with the video settings at max as well. I believe Thomas was right, just a missing game file, though the game wouldn't work until I deleted everything in My Docs and tried it again.
