22/09/2014 -> atikmpag.sys (AMD Drivers)
24/09/2014 -> atikmpag.sys (AMD Drivers)

You need to do a clean reinstall of the drivers. Before we proceed, what operating system do you have? Also x86 (32-bit) or x64 (64-bit)?
