In a lovecraftian small town.
You'd start walking around and seeing normal sights (human npcs interacting with the player and each other, kids playing, bustling atmosphere, etc etc etc) and when night hits it turns sinister.
The protagonist could wander into a church, which then connects into a cave that has subtle signs of some otherworldly stuff. The cave connects to other landmarks in the town and to get deeper into the cave system you have to explore for alternate entries and exits using gear ( a la metroid). So it'd have a lot of backtracking and ''free form'' exploration.
Oh God. Someone steal my idea and make it happen. Oh no, "free form" exploration and a ton of useless NPCs make it feel like an abandoned MMO. I hate NPCs.
The npcs would just be there at the beginning to give off the vibe that there's an actual town and people there. So the player can more precisely measure how quickly stuff turned to shit.
As for the free form exploration it'd work. It'd still have to be linear in certain ways cause it's a horror game but it'd be a bit more free form than the Penumbras and Amnesias.

Hm...things I'd like to see in a new Frictional game (Amnesia or otherwise):
-More intelligent enemies. Zombies and Pig-Men may have an excuse for being idiots, but I personally find humans scary because they're smart bastards.
-Giving the protagonist a body ala Outlast
-Seeing and interacting with actual NPCs ala Half-Life

You know that there was no "Team Silent" as in: The same people for the first four Silent Hill games? There was a "Team Silent Hill 1/2/3/The Room" with only three people being fixtures to all of these. One of them being Akira Yamaoka who quit the franchise after Shattered Memories.
On Topic: Most of these ideas are pretty scary, but not in a good way...

FEAR did that back in 2005 - all the better to see you kick a bad guy's face in.

OK, this is just a concept, but here I go:
Both Daniel's and Mandus' families fatefully merge at some point, and you play as Klaus, a young man with a good reputation, but then he learns about the darkness of his families' pasts.
You then find where the Machine and the Orb are hidden, but not only are the Shadow and the Pigmen after you, but you also have a secret organisation that call themselves "the Peacekeepers" after you.
That's all I got for now, but I will come back with more

The Peacekeepers are a secret organisation that find and contain weird artifacts and machines in an effort to gain a better understanding of them. Klaus is a descendant of both Daniel and Mandus, looking for the Orb and the Machine to try to destroy them, so that they cannot be used for evil purposes.
However, since Klaus is "in the way" the Peacekeepers have "no choice" but to take him out of the picture. What neither realises is the danger behind the objects they are trying to obtain or destroy, so, while the Peacekeepers are smart, they know not of the powers of the Orb or the Shadow that guards it, they only know about the Servant Grunts.
