Prison, Last part of sewer and the Choir.
The prison for the massive amount of monsters.
The end of the sewers where you have to bait the brute was nerve wrecking.
The Choir because when a monster was on you I never had a clue from what direction he came. Also those creepy torture rooms. The Iron Maiden made me jump so high.

The choir is by far the creepiest thing in the game.
The chancel and the prison area are also parts that scared the hell out of me.

Coming out of the Archives when you first encounter the Shadow, I did not expect it to attack me. I actually yelled because I thought a monster had appeared and was attacking me; when I realized how stupid I was, I just laughed.
Other than that, both parts of the Prison - but especially the frickin' North Block - had me so tensed up I could barely move for a while. Serious triumph of atmosphere.

The choir. Ohhh man, I walked in, and I just sat there for a minute and said, "No. No, no, nonononononono." Everything was that sickly blood red color, way too creepy. Then I hear Mr. Tall start stalking around and I just sat in a corner and cowered for about five minutes before continuing on.
The most STARTLING part must've been when I opened the door to the sanctum and the Mr. Face brigade came out of nowhere and trucked me before I even realized what the hell just happened.

Dark Storage, scariest part ever.
I opened that door where the monster waits in the room. I didn't scream, but I nearly fell off my chair. I left my room and refused to go back for a couple hours. Then I quit for a week.
