[Spoilers] Overture Blank Note

I have been playing overture and i seem to be missing the note you need to put underneath the fluorescent light to get the door code.

I don't remember seeing this note before... any ideas?

If you were in Shaft 13, there was a locker with the note behind a mining helmet in a locker.
