Ideally, they don't spawn at all. They are either near the player or somewhere else but don't magically appear and disappear. Technically, it makes sense to simply spawn them somewhere but this mechanic shouldn't be obvious to the player and the player shouldn't have to wonder e.g. how an enemy appeared or disappeared with all doors locked etc.
