Maybe then another program is causing problems like an antivirus or similar. You can check some program that cause problems at the troubleshooting guide but these are only those reported by this forum

Tried disabling MSE, turning off the Afterburner OSD, etc. Didn't seem to help either.

After verifying game cache via steam, run the game until it lags and attach the HPL.log file again.

Alright, here is my latest HPL log. Everything on max (SSAO samples set to 16, using FXAA via the CP instead of the traditional AA option, playing at a 1920 X 1080 resolution, "prefer maximum performance" also set in the CP). I just ran to the location where you assemble the orb and there was a good deal of stuttering, particularly when opening the first door to enter the main room, when the monster appears and walks away, etc.

The logs seems just fine and your have 55fps which is excellent, only one file cannot be found. Maybe another steam verification will solve it. Other than that, everything looks good as far as the game is concerned.

Maybe the drivers or another program is causing this issue...

Open Documents/Amnesia/Main/main_settings.cfg for editing. Make sure LimitFPS is false and Vsync is false. Save the changes, restart the game. Better?

Driver sweeper and a clean install non beta this time just 306 driver would be ok.
As I don't have a gtx 670 I don't know it it is just you so that is why I advice on a clean driver install.

I have a gtx 570 and a hd7850 LP though

Removing the FPS limiter and turning V-sync off didn't help.

I have a USB 3.0 flash drive with good read/write times; I'll try installing the game on that instead of my mechanical HDD.

If you have Steam version, it may not launch when it's not in the steamapps folder.

Don't lower any graphics setting. Your card is powerful enough!
Except the SSAO Samples shouldn't set higher than 16. Because they will slow down performance enormous! Let SSAO Samples set to 16.
But try to disable V-Sync and then go to graphic driver options and check "Multi display performance mode" is set to "Single display performance mode".
