Most likely, it's not the monster's head.

Even if it is, Frictional Games' most of the released screenshots (from days of Penumbra) are either not in the game, or modified from actual in-game environment by adding small stuff.

I dunno why, but it looks like the monster might be communicating with another down the alley. It kinda looks like it dropped the person it was dragging to turn and talk (if that's it's right arm)

I hope they have more monster variety so we don't get used to 1-3 faces (even though they're still scary as shit, but just saying).

I think the body that it's dragging looks a little behind it because it is using a meat hook to drag the corpse.
