Yes, not only do you need a new game but you also need a new profile.
I just tested this: If I try to load the save from the profile that I made the first video of ( in this post) then the stutter is back even though I'm using the patched exe, patched dll and the new main_settings.cfg values.
I am not using any of the extra files provided in aamfp_stutter3.zip. The only ones I need to fix my game are in aamfp_stutter4.zip. I was using these values though: MaxChannels="48" StreamBuffers="6" StreamBufferSize="32768"

EDIT: As stated in the next post, deletion isn't necessary if you verify the game cache integrity.
The slightly tedious way would be to manually delete all the files provided in the zip file from your installation directory.
Alternatively, you could do the heavy-handed approach and delete everything in /sounds/ambience/ + sounds/events/ + /maps/ (Warning: Doing it this way will make you have to download several gigs worth of game files again.)
You can redownload the original files by verifying the integrity of your game cache as plutomaniac suggested (in Steam go to Library -> Right click the game -> Properties -> Local Files -> Verify integrity of game cache).
You should always make a backup copy of all the original files you are replacing before trying new patches out so that you won't have to manually delete or redownload stuff. You would then just be able to copy+paste all your original files back.

Just verify integrity of files using steam. It will tell you 152 files will be fixed and that's all. Afterwards, apply the stutter5 fix.

I'm using patch4, with high preset and the streambuffer edit - it runs much better now, although it's crashed once so far, which happened after selecting 'Save and quit' from an area. However, it did indeed save.

In desperate hope I decided to install the game on my SSD (it reads at 500 MB/s), installed test patch4 and.... it stutters. I turned off every graphics option and set the resolution to 640x480 and... it still stutters.
The game is playable, but the stuttering is so annoying that it completely destroys the atmosphere of the game. Maybe it is a driver problem, but I seriously don't feel like installing older drivers to see if it works better.
OS: Win7 x64
GPU: HD5770, 1 GB (Catalyst 13.10 Beta)
CPU: Phenom II X4 @ 3,2 GHz
RAM: 4 GB

Don't use Catalyst 13.10 Beta but try the latest stable driver from AMD which is 13.4 as of now. Also, it's not enough to apply stutter4 fix. You also need to change some sound settings. So, install stable 13.4 catalyst and try stutter5 fix as I mentioned above some times.

Terrible Framerate even on lowest settings
I started up the game, and tried high settings, just to see how it would be. As I expected, the framerate was low. I then put it on medium thinking I was gonna go on my merry way, and the framerate was almost as bad as it was before. That's strange, I figured. I put it on low and its the same. I gave up trying and eventually played the game through until the Sewers level and got tired of it. Is there any way I can fix the abysmal framerate? Here is my computer specs:
Intel ® Core I7-2630QM CPU @ 2.00 GHz
8 GB of Ram
64 bit
Windows 8
DirectX 11
Aaaand what I believe to be the source of this low framerate, Graphics Card: Nvidia GeForce GTX 460M.
If anyone could help me out, that'd be great. I am confused at why the framerate is terrible; I played Metro: Last Light on Ultra settings with half the lag I have when I play AMFP on lowest settings.
